Comorbidités médicales chez les enfants et les adolescents avec un diagnostic de "troubles du spectre de l'autisme" et des troubles de l'hyperactivité déficitaire de l'attention: une revue systématique

Les troubles somatiques se produisent plus souvent chez les patients adultes suivis en psychiatrie que dans la population adulte générale.
Cependant, dans la psychiatrie de l'enfant et de l'adolescent, cette association n'est pas claire, principalement en raison d'un manque d'intégration des données existantes. Pour aborder cette question, les chercheurs présentent une revue systématique de la comorbidité médicale dans les deux troubles majeurs du développement, le "trouble du spectre de l'autisme" (TSA) et le trouble déficitaire de l'attention avec hyperactivité (TDAH) et ils formulent des recommandations cliniques.
La recherche initiale a identifié 5278 articles et a produit 29 études pour inclusion. Cette recherche approfondie de la littérature donne un aperçu des articles pertinents sur la comorbidité médicale dans le TDAH et/ou TSA, et montre que les troubles médicaux chez ces enfants et les adolescents semblent être répandus.  
Ceux qui travaillent avec des enfants avec un diagnostic de TSA et/ou de TDAH devraient être conscients de cela et promouvoir activement l'évaluation médicale de routine. En outre, des études de cas-témoins et des études basées sur la population sont nécessaires pour fournir des estimations de prévalence fiables. Les études futures devraient également se concentrer sur une évaluation plus large des troubles médicaux chez les enfants et les adolescents avec un diagnostic de TDAH et/ou TSA pour améliorer l'algorithme de traitement dans ce groupe vulnérable.

Abstract

Somatic disorders occur more often in adult psychiatric patients than in the general adult population. However, in child and adolescent psychiatry this association is unclear, mainly due to a lack of integration of existing data. To address this issue, we here present a systematic review on medical comorbidity in the two major developmental disorders autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD) and formulate clinical recommendations. The literature was searched using the PubMed and PsycINFO databases (2000-1 May 2016) with the keywords "[((child and adolescent) AND (Autism OR Attention Deficit Hyperactivity Disorder* OR ADHD)) AND ("Cardiovascular Diseases" [Mesh] OR "Endocrine System Diseases" [Mesh] OR "Immune System Diseases" [Mesh] OR "Neurobehavioral Manifestations" [Mesh] OR "Gastrointestinal Disorders" [Mesh] OR Somatic OR Autoimmune disease OR Nervous system disease OR Infection OR Infectious disease)]. Two raters independently assessed the quality of the eligible studies. The initial search identified 5278 articles. Based on inclusion and exclusion criteria 104 papers were selected and subsequently subjected to a quality control. This quality was assessed according to a standardized and validated set of criteria and yielded 29 studies for inclusion. This thorough literature search provides an overview of relevant articles on medical comorbidity in ADHD and/or ASD, and shows that medical disorders in these children and adolescents appear to be widespread. Those who work with children with ASD and/or ADHD should be well aware of this and actively promote routine medical assessment. Additionally, case-control studies and population-based studies are needed to provide reliable prevalence estimates. Future studies should furthermore focus on a broader evaluation of medical disorders in children and adolescents with ADHD and/or ASD to improve treatment algorithm in this vulnerable group.

[Profil communicatif et social-adaptatif chez les enfants avec un diagnostic de troubles du spectre de l'autisme: une nouvelle approche basée sur les critères DSM-5].

Suite à l'adoption de la nouvelle classification internationale du diagnostic à partir du Manuel de diagnostic et statistique (DSM-5), le trouble du spectre de l'autisme (TSA) a été établi comme une catégorie dimensionnelle qui comprend d'autres troubles qui étaient auparavant considérés comme des entités distinctes. Des recherches antérieures ont montré que certaines personnes diagnostiquées avec ce trouble présentent différents profils communicatifs et linguistiques. Par conséquent, des résultats contradictoires peuvent être trouvés chez les personnes qui reçoivent le même diagnostic.
L'objet de cette étude était de distinguer les aspects du langage structurel (expression et compréhension), les aspects interactifs (pragmatiques) et l'adaptation sociale entre les enfants ayant un TSA avec un niveau 1 de soutien et les enfants ayant un développement typique. 
Dix-sept enfants avec un diagnostic de syndrome d'Asperger (selon DSM-IV-TR) et 20 enfants ayant un développement typique entre 7 et 12 ans ont participé à l'étude. Les chercheurs ont assimilé le diagnostic du syndrome d'Asperger au TSA avec un niveau 1 de soutien. 
Ils ont évalué le quotient intellectuel, la communication et l'adaptation sociale avec des échelles parentales standardisées directes et indirectes et ils ont trouvé des différences significatives dans la compréhension (p = 0,025), l'interaction (p = 0,001) et l'adaptation sociale (p = 0,001) entre les deux groupes. 
Les sujets ayant un diagnostic de TSA et un niveau 1 de soutien démontrent un QI moyen et une bonne structure expressive (syntaxe et niveau sémantique), qui peut être différente des autres enfants qui reçoivent le même diagnostic, en raison de la grande hétérogénéité du trouble. Néanmoins, nos sujets ont des problèmes liés à la compréhension de la structure grammaticale, de la pragmatique et de l'adaptation sociale. Ces difficultés peuvent être liées à des problèmes émotionnels et sociaux.

Abstract

INTRODUCTION:

Following the adoption of the new international diagnosis classification from the Diagnostic and Statistical Manual (DSM-5), autism spectrum disorder (ASD) has been established as a dimensional category that includes other disorders that were previously considered as separate entities. Previous research has shown that some people with this disorder exhibit different communicative and linguistic profiles. Therefore, contradictory results could be found among people who receive the same diagnosis.

AIM:

To distinguish structural language aspects (expression and comprehension), interactive aspects (pragmatics), and social adaptation between children with an ASD-level 1 of support and children with typical development.

SUBJECTS AND METHODS:

Seventeen children with Asperger syndrome (according to the DSM-IV-TR), and 20 children with typical development between 7 and 12 years old. We have equated diagnosis of Asperger syndrome with ASD-level 1 of support. We have evaluated intelligence quotient, communication, and social adaptation with direct and indirect standardized parental scales.

RESULTS:

We have found significant differences in comprehension (p = 0.025), interaction (p = 0.001), and social adaptation (p = 0.001) between the two groups.

CONCLUSIONS:

Subjects with ASD-level 1 of support demonstrate an average intelligence quotient, and good expressive structure (syntax and semantic level), which may be different from other children who receive the same diagnosis, due to the wide heterogeneity of the disorder. Nevertheless, our subjects have problems related to comprehension of grammar structure, pragmatics, and social adaptation. These difficulties could be related to emotional and social problems.

Bien qu'une énorme quantité de recherche ait été faites sur les compétences de conversation déficientes chez les personnes avec un diagnostic de "troubles du spectre de l'autisme" (TSA), on sait peu de choses sur leur performance sur les présupposés, un domaine de connaissance crucial pour une communication réussie.Cette étude a étudié la compréhension de quatre types de présupposition, à savoir les présupposés existentiels, facultatifs, lexicaux et structurels, chez des enfants de langue cantonaise d'âge scolaire "avec et sans TSA".  Un groupe d'enfants avec un diagnostic de TSA (n = 21), âge moyen 8,8, a été comparé à un groupe d'enfants au développement typique (n = 106).
Les enfants avec un diagnostic de TSA ont montré un déficit dans la compréhension des présupposés, même après avoir contrôlé les différences dans la capacité linguistique générale et l'intelligence non verbale. La difficulté relative des quatre types de présupposition ne différait pas entre les deux groupes d'enfants.

Comprehension of presuppositions in school-age Cantonese-speaking children with and without autism spectrum disorders

Abstract

While an enormous amount of research has been done on the deficient conversation skills in individuals with autism spectrum disorders (ASD), little is known about their performance on presuppositions, a domain of knowledge that is crucial for successful communication. This study investigated the comprehension of four types of presupposition, namely existential, factive, lexical and structural presuppositions, in school-age Cantonese-speaking children with and without ASD. A group of children with ASD (n = 21), mean age 8.8, was compared with a group of typically developing children (n = 106). Knowledge of presuppositions was evaluated based on children's ability to judge whether a given utterance was a correct presupposition of a preceding utterance. Children with ASD were found to show a deficit in the comprehension of presuppositions, even after controlling for differences in general language ability and non-verbal intelligence. The relative difficulty of the four types of presupposition did not differ between the two groups of children. The present findings provide new empirical evidence that children with ASD have a deficit in the comprehension of presuppositions. Future research should explore whether the deficit in the comprehension of presuppositions is related to the development of theory of mind skills in children with ASD.

Mov10 supprime les rétroéléments et régule le développement et le fonctionnement neuronal dans le cerveau en développement

Le virus de la leucémie de Moloney 10 (Mov10) est une hélicase (source Wiki: protéines qui utilisent l‘énergie d’hydrolyse de l’ATP ou du GTP pour catalyser l’ouverture d’acides nucléiques (ADN ou ARN) appariés sous forme double brins. ) d'ARN qui intervient sur l'accès du complexe RISC aux ARN messagers (ARNm).  
Mov10 est essentiel pour le développement neuronal normal et la fonction du cerveau. Mov10 se lie préférentiellement aux ARN impliqués dans la liaison de l'actine, la projection neuronale et le cytosquelette. Il s'agit d'une fonction complètement nouvelle et extrêmement importante pour Mov10 dans le développement neuronal et établit un précédent pour Mov10 être un candidat important dans les troubles neurologiques qui ont des causes cytoarchitecturales sous-jacentes comme l'autisme et la maladie d'Alzheimer.

Abstract

BACKGROUND:

Moloney leukemia virus 10 (Mov10) is an RNA helicase that mediates access of the RNA-induced silencing complex to messenger RNAs (mRNAs). Until now, its role as an RNA helicase and as a regulator of retrotransposons has been characterized exclusively in cell lines. We investigated the role of Mov10 in the mouse brain by examining its expression over development and attempting to create a Mov10 knockout mouse. Loss of both Mov10 copies led to early embryonic lethality.

RESULTS:

Mov10 was significantly elevated in postnatal murine brain, where it bound retroelement RNAs and mRNAs. Mov10 suppressed retroelements in the nucleus by directly inhibiting complementary DNA synthesis, while cytosolic Mov10 regulated cytoskeletal mRNAs to influence neurite outgrowth. We verified this important function by observing reduced dendritic arborization in hippocampal neurons from the Mov10 heterozygote mouse and shortened neurites in the Mov10 knockout Neuro2A cells. Knockdown of Fmrp also resulted in shortened neurites. Mov10, Fmrp, and Ago2 bound a common set of mRNAs in the brain. Reduced Mov10 in murine brain resulted in anxiety and increased activity in a novel environment, supporting its important role in the development of normal brain circuitry.

CONCLUSIONS:

Mov10 is essential for normal neuronal development and brain function. Mov10 preferentially binds RNAs involved in actin binding, neuronal projection, and cytoskeleton. This is a completely new and critically important function for Mov10 in neuronal development and establishes a precedent for Mov10 being an important candidate in neurological disorders that have underlying cytoarchitectural causes like autism and Alzheimer's disease.

Utilisation pathologique du jeu chez les adultes "avec et sans" "trouble du spectre de l'autisme"

Cette étude a permis de vérifier si les adultes avec un diagnostic de "troubles du spectre de l'autisme" (TSA) courent un risque plus élevé d'utilisation pathologique des jeux que les adultes avec un développement typique (TD).
Les résultats indiquent que les adultes avec un diagnostic de TSA ont éprouvé plus de symptômes de la pathologie des jeux vidéo que les adultes TD. 
Les résultats ont également montré que les adultes  avec un diagnostic de TSA ont passé plus d'heures quotidiennes à jouer à des jeux vidéo et ont consacré plus de leur temps libre à jouer à des jeux vidéo que les adultes TD. Même après l'ajustement pour ces différences dans les heures quotidiennes de jeux vidéo et la proportion de temps libre consacré aux jeux, les comparaisons de modèles ont révélé une différence dans les scores de pathologie du jeu associés au statut TSA.   
En conclusion, le risque d'utilisation pathologique du jeu semble plus important chez les adultes avec un diagnostic de TSA par rapport aux adultes TD .

Pathological game use in adults with and without Autism Spectrum Disorder

Abstract

This study tested whether adults with autism spectrum disorder (ASD) are at higher risk for pathological game use than typically developing (TD) adults. Participants included 119 adults with and without ASD. Participants completed measures assessing daily hours of video game use, percent of free time spent playing video games, and symptoms of pathological game use. The results indicated that adults with ASD endorsed more symptoms of video game pathology than did TD adults. This relationship was strong, enjoying 300,000-to-1 odds in Bayesian model comparison. Results also showed that adults with ASD spent more daily hours playing video games and spent a higher percent of their free time playing video games than did TD adults. Even after adjustment for these differences in daily video game hours and proportion of free time spent on games, model comparisons found evidence for a difference in game pathology scores associated with ASD status. Additionally, escapism motives for playing video games was associated with game pathology scores in both ASD and TD adults, replicating and extending a previous report. In conclusion, the risk for pathological game use appears larger in adults with ASD compared with TD adults. These findings point to pathological game use as a potentially important focus of clinical attention in adults with ASD.

Abstract

Les enfants avec un diagnostic de TSA ont été caractérisés par des niveaux significativement plus élevés de cuivre (Cu) (+ 8%), de fer (Fe) (+ 5%) et de sélénium (Se) (+ 13%) dans les cheveux et seulement 8% de plus de sérum de Cu. Après la stratification pour le genre, les garçons avec un diagnostic de TSA se caractérisaient par une augmentation significative des niveaux de Cu (+ 25%), de Fe (+ 25%) et de Se (+ 9%), alors que chez les filles, seul le contenu de SE était élevé (+ 15%).
Les garçons et les filles avec un diagnostic de TSA ont été caractérisés par plus grand niveau sérique de manganèse (Mn) significativement plus élevé (+ 20%) et de Cu (+ 18%) par rapport aux valeurs des témoins.
Dans le groupe des enfants plus jeunes (2-5 ans), aucune différence significative de groupe dans les niveaux d'oligo-éléments des cheveux n'a été détectée, alors que les concentrations sériques de Cu étaient significativement plus élevées (+ 7%).  
À son tour, la concentration sérique de Se chez les enfants avec un diagnostic de TSA était inférieure de 11% à celle des enfants neurotypiques. Dans le groupe des enfants plus âgés avec un diagnostic de TSA (6 à 10 ans), les niveaux de cheveux Fe et Se étaient de 21% et 16% plus élevés, alors que dans le sérum, les niveaux de Cu ont augmenté (+ 12%) par rapport aux témoins. L'analyse de corrélation a également révélé une relation différente entre les niveaux des oligo-éléments du sérum et des cheveux par rapport au genre et à l'âge.  
Par conséquent, il est fortement recommandé d'évaluer plusieurs matrices bioindicatives pour l'évaluation critique de l'état des oligo-éléments chez les patients avec un diagnostic de TSA afin de développer une correction nutritionnelle personnalisée adéquate. 
The primary objective of the present study was to investigate the levels of essential trace elements in hair and serum in children with autism spectrum disorder (ASD) and investigate the age and gender effects. Children with ASD were characterized by significantly higher levels of copper (Cu) (+8%), iron (Fe) (+5%), and selenium (Se) (+13%) levels in hair and only 8% higher serum Cu levels. After stratification for gender, ASD boys were characterized by significantly increased hair Cu (+ 25%), Fe (+ 25%), and Se (+ 9%) levels, whereas in girls only Se content was elevated (+ 15%). Boys and girls suffering from ASD were characterized by significantly higher serum manganese (Mn) (+20%) and Cu (+18%) as compared to the control values, respectively. In the group of younger children (2-5 years), no significant group difference in hair trace element levels was detected, whereas serum Cu levels were significantly higher (+7%). In turn, the serum concentration of Se in ASD children was 11% lower than that in neurotypical children. In the group of older children with ASD (6-10 years), hair Fe and Se levels were 21% and 16% higher, whereas in serum only Cu levels were increased (+12%) as compared to the controls. Correlation analysis also revealed a different relationship between serum and hair trace element levels with respect to gender and age. Therefore, it is highly recommended to assess several bioindicative matrices for critical evaluation of trace element status in patients with ASD in order to develop adequate personalized nutritional correction.

Participation des enfants "dans le spectre de l'autisme" à la maison, à l'école et dans la communauté

Cette étude visait à documenter les modèles de participation et les opinions des soignants en ce qui concerne la fréquence et l'intensité des activités auxquelles participent les enfants avec un diagnostic de "trouble du spectre de l'autisme".
Les aidants naturels des enfants âgés de 5 ans (n ​​= 90) et de 9 à 10 ans (n ​​= 128) ont complété le questionnaire sur la mesure de la participation et de l'environnement pour les enfants et les jeunes pour la maison, l'école et la communauté.  
Les analyses ont révélé des modèles de participation similaires à la maison, à l'école et dans la communauté pour les deux cohortes avec des différences précoces en fonction de l'âge. Abstract

BACKGROUND:

Children on the autism spectrum participate less frequently, and in a narrower range of activities, than their nonautistic peers, but little is known about exact participation patterns across contexts or how this is perceived by caregivers. This study aimed to document patterns of participation and caregiver views with regard to frequency and intensity of activities.

METHOD:

Caregivers of children on the spectrum aged 5 (n = 90) and 9-10 years (n = 128) completed the Participation and Environment Measure for Children and Youth for home, school, and community. Caregivers reported on frequency of child's participation, level of involvement, and caregivers' desire for change in participation patterns.

RESULTS:

Item-level analyses revealed similar patterns of participation across home, school, and community for both cohorts with some small age-appropriate differences. Caregivers generally desired increased diversity, frequency, and involvement in activities but a decreased use of electronics (computers, games, TV, and DVDs).

CONCLUSION:

The possibility of autism-specific participation patterns could inform future interventions aimed at enhancing social inclusion. This warrants further investigation through multiinformant designs that seek the perspectives of the child and caregivers.

DHA alimentaire, bioaccessibilité et développement neurobiologique chez les enfants

Le DHA est un acide gras polyinsaturé n-3 nutritionnel clé (AGP n-3) et doit être apporté par l'alimentation humaine.
Des niveaux élevés d'apport en DHA semblent réduire le risque de dépression, de trouble bipolaire et de troubles de l'humeur. Sur la base de ces connexions entre le DHA et la santé neurologique, cet article examine ce qui est actuellement connu au sujet du DHA et du développement neurologique  des enfants, ainsi que les avantages de l'absorption de DHA dans la prévention de l'autisme et des troubles du comportement.

Abstract

DHA is a key nutritional n-3 polyunsaturated fatty acid (n-3 PUFA) and needs to be supplied by the human diet. High levels of DHA intake appear to reduce the risk of depression, bipolar disorder, and mood disorders. On the basis of these connections between DHA and neurological health, this paper reviews what is currently known about DHA and children neurodevelopment as well as the benefits of DHA intake to prevention of autism and behaviour disorders through a selective and representative revision of different papers ranging from pure observational studies to randomized controlled trials (RCTs). This review also highlights the issue of DHA bioaccessibility and its implications to the performance of studies. As main conclusions, it can be mentioned that high DHA intake may prevent autism disorder. However, more studies are required to strengthen the connection between autism and dietary DHA. Regarding behavioural disorders, the evidence is also contradictory, thereby raising the need of further studies. From all screened studies on autism, ADHD, and other disorders, it can be concluded that study samples should be larger for greater statistical significance and RCTs should be more carefully designed.

Cette étude est un compte rendu réfléchi de la façon dont la résolution des problèmes a été réalisée  lors de la traduction d'entrevues semi-structurées d'une langue source à une langue cible. Les données proviennent de deux études de recherche qualitative dans lesquelles l'analyse phenoménologique interprétative a été utilisée pour obtenir des informations sur l'expérience vécue par les parents d'enfants handicapés en Inde et en Turquie. Les auteurs discutent des problèmes de l'interprétation qui se produisent lorsque les participants et le chercheur principal parlent la même langue maternelle non anglaise et que les résultats de l'étude sont destinés à un public anglophone.

Abstract

This study is a reflective account of how problem solving was accomplished during the translation of semi-structured interviews from a source language to a target language. Data are drawn from two qualitative research studies in which Interpretative Phenomenological Analysis was used to obtain insights into the lived experience of parents of children with disabilities in India and Turkey. The authors discuss challenges to interpretation that arise when participants and the main researcher speak the same non-English native language and the results of the study are intended for an English-speaking audience. A common theme in both the Turkish and Indian data relates to parents' understanding of their children's symptomology and the prognosis. Implications include the need for both reflective conversation within the research team to address the translation of problematic utterances, and documentation of the translation process in the presentation of research findings.
